Important notes:

Not all facilities can be reserved through this form:
- Sam Lynn Ballpark reservations are handled by the Pecos League
- Lamont Park Ball Field reservations can be done at the Dave Head Building at 10300 San Diego Street, Lamont CA
- Otherwise, if the facility you’re looking for is not listed on the form then contact Parks
Reservation lead times:
- Neighborhood Park Picnic Shelters: can reserve up to 2 weeks in advance
- Area Park Group Picnic Areas, Mountain Camps, and Playing Fields: can reserve up to 30 days in advance.

Insurance information does not need to be included in the submission, as it will be requested later in the process.
